Mary Ann Yuhanek passed away on September 8, 2015 at Archbold Memorial Hospital. Born May 28, 1945 in Cleveland, Ohio, she was the daughter of the late Michael John and Julia Yuhanek. She was married to George Edward Cook, who preceded her in death. Survivors include daughter Ruby Ann Williams (Vincent); grandchildren, Tanya Marie Rivera (Alberto) and Candace Ann Lagunes (Michael Hines); great grandchildren Alberta Jorge Rivera, Angel Aikmar Lagunes, Luis Enrique Rivera, Alejandro Elias Rivera, Francisco Antonio Salinas, Ricardo Emannual Rivera; and sister Josephine McQuillen.
